In accordance with Texas Department of State Health Services, health screenings will continue to be required for the school year 2020-2021. Parents may choose to provide health screenings from their own health care provider.
I will conduct screenings for students who are present on campus and curbside appointments will be available for those who are doing virtual instruction. Please call to schedule your appointment today.
The screening forms are attached below to be filled out by your provider and may be dropped off at the campus or returned via fax at 956-323-8294 or email [email protected].
If your child will be coming to campus, please fill out and sign the Parent Acknowledgment Form, prior to entry. We can provide the form if you are unable to print it.
